EBay Fees Calculator

I tried with eBay Fees Calculator and I am subject to some enhancements as following:
1. Integrated this tool into a draft page. (edit variant)
2. I need more fields:
- Additional URL field for source product
- Sales tax (vary by state in the USA)
- eBay ad rate
- Topup fee for Fulfilled by AutoDS: Paypal (4%)/Payoneer(1%)
- Paypal fee: 2.9% + $0.3
